Requirements for Home School
Two forms are required to be submitted to the Houston County Board of Education; the Declaration of Intent and the Monthly Attendance Record.
Fill out the Declaration of Intent form, including your name, address, the names of your children you intend to home school, their ages and the dates you intend to school. Make a copy for your files.
Mail or bring in the Declaration of Intent by September 1st, or within 30 days of the establishment of your homeschool program to:

***The Declaration of Intent is required by the state of Georgia and has to have an original signature. This cannot be emailed or faxed. Please send by regular mail or bring it in. ****

Monthly Attendance Records need to be sent by the 5th day of every month to the address above or you may email them to: homeschool@hcbe.net. Keep a copy of each Attendance Form for your files.

Forms are fillable, however you must have the current verison of Adobe Acrobat Reader which is a free download at Free Adobe Reader
Instructions for emailing Monthly Attendance Records
When submitting your attendance record, please do the following:
1. Save the document on your computer using the name of the child/children and the month, e.g., Green, Kimberly-Sept. If you have more than one child, please use all children’s names, and list in alphabetical order according to last names.
2. Once you have completed the monthly attendance record and saved the document correctly, you may now attach it to an email. Please put the student’s name in the subject line (last name, first name - month) and send to:
homeschool@hcbe.net
3. DO NOT copy and paste in the email. Please send as an attachment! This email account is strictly for Attendance Records. All questions should be forwarded to Kimberly Green.
You may send your attendance records via regular mail if you do not want to email.
