HHB for Student Services Website


Parents or legal guardians applying for Hospital Homebound services for their child should complete the following procedure:

1. Contact the student’s home school and schedule a conference with the Hospital Homebound liaison. The liaison will go over the application and process with the parent. Part I and Part II of the application will be completed at this conference.
2. Arrange for a certified physician to complete Part II of the application.
3. Contact the Hospital Homebound clerk to schedule an appointment at the BOE to review the completed application with the parent and sign the back of the form indicating understanding of parent responsibilities.
4. The application will be placed on schedule of the first available Hospital Homebound Review Committee meeting.

FAQ

1. What is HHB?
HHB services are provided for approved Houston County public school students who have a medically diagnosed physical condition restricting the student to home or a hospital for a period of time and significantly interferes with the student’s education
2. Is my child eligible?
Students must be enrolled in a Houston County public school and submit a completed application to be considered for services.
3. Who provides the service?
Certified teachers provide the service.
4. Where is the service provided?
Services may be offered in the home, hospital, public library, at the
school outside of the regular school day or other mutually agreeable place.
4. How often will my child meet with the teacher?
The delivery model provides for 3 hours of instruction for every 5 days
of student absence.
