HHB for Student Services Website
Hospital Homebound (HH) provides academic instruction to students who are confined either at home or in a health care facility for periods of time that would prevent normal school attendance. We provide academic instruction based on certification of need by a licensed physician or licensed psychiatrist who is treating the child.
HH is for a child that has a medical diagnosis that restricts school attendance for 10 consecutive days or has a chronic illness that restricts attendance for 10 or more days over a period of time. If this applies to your child:

Contact your child’s school and speak to the HH Liaison. She/he can give you an application that must be completed by the Liaison, you and your child’s physician.

The form is turned in to the HH Clerk at the Houston County Board of Education office in Perry, GA. The HH Review Committee meets twice a month. They will review the application and make a recommendation of services to the Assistant Superintendant of Student Services. You are welcome to attend the committee meeting. A specific time can be scheduled through the HH Clerk. A letter will be sent to you informing you of the committee recommendation/decision.

If approved, a HH service provider (teacher) will be assigned. They will contact you and schedule a time to begin services. An adult, 21 years of age or older, must be in attendance during instructional services.

Continue to pick up work for your child until you have been contacted by the assigned service provider.

Absences due to the approved diagnosis and within the dates requested by the physician will be covered based on hours of service provided.
