Perry Middle School Athletics Requirements

Age
7th Grade – Student must be under 14 years of age before May 1st of their 7th grade year. 7th graders that turn 14 before May 1st are eligible for 8th grade athletics.
8th Grade - Student must be under 14 years of age before May 1st of their 8th grade year. Players turning 16 before May 1st are deemed ineligible for middle school athletics.
Physicals
Student must have an up-to-date physical on file with PMS. Physicals are good for one year from the date they are signed by the student's doctor.
Insurance
Student must provide verification of insurance on their physical form. Players without family insurance must purchase school insurance before participating.
Parental Consent
Student must have the parental consent portion of their physical form filled out and signed before they will be allowed to participate.
Academics
Eligibility for 1st Semester: 1) Student must have been promoted to the next grade. (Attending Summer school can meet this requirement); 2) Student must have passed all 4 academic classes for the school year. (Attending Summer school can meet this requirement); 3) Student must have passed 5 out of 6 classes the previous semester. (Attending Summer school can meet this requirement); 4) If retained, student will be ineligible for 1st semester athletics.
Eligibility for 2nd Semester: Student must have passed 5 out of 6 subjects; student does not have to pass all 4 academics.
Additional Eligibility Procedures
A) Any student who enters the 8th grade and is in an 8th grade homeroom for the 3rd year or semester is ineligible. B) A student can participate on an 8th grade team for 2 years provided that one of those years the student was assigned to a 7th grade homeroom. C) An athlete will only be allowed to participate in middle school athletics for 2 years provided that they meet all age and academic requirements. D) An athlete can only participate in one sport per season. E) Cheerleaders cannot play softball or basketball; baseball players cannot run track.
