Kindergarten/Pre-K Registration
Houston County Elementary Schools
March 3 - 14, 2008
9:00 a.m. - 3:00 p.m.
Required for Registration:
Kindergarten
* Copy of Official Birth Certificate
* Ear, Eye, and Dental Screening Form 3300
* Immunization Certificate Form 3231
* Social Security Card
* Proof of Residency Required
*Kindergarten students must be 5 years old on or before September 1.
Pre-Kindergarten
* Copy of Official Birth Certificate
* Social Security Card
* Proof of Residency required
All Children Eligible
Applicants will be entered into computer system for lottery drawing. All applicants will be notified by April of acceptance or waiting list status.
If eligible for Medicaid, SSI, TANF, PeachCare, food stamps, you must bring proof of eligibility
Parents are expected to participate in parent workshops and monthly parent activities
NEEDED BUT NOT REQUIRED AT PRE-K REGISTRATION:
* Ear, Eye, and Dental Screening Form 3300
* Immunization Certificate Form 3231
Students should register in their current school zones.
Students currently enrolled in public Pre-K programs do not need to register again.

Dear Parent/Guardian:
Welcome to the Houston County School System! Our goal is to provide your child with the best education possible. Your involvement in the process is actively sought.
As with all school systems, there are certain state and local requirements, as well as documentation required for enrollment and the proper placement of your child. Your responsibility is to provide the following documentation within the specified time limits.
1. Birth Certificate (Grades Pre-K, K and 1) ¡V At Enrollment
2. Certificate of Immunization (Georgia Form 3231) ¡V At Enrollment
Out-of-state students or initial student enrollments must submit Georgia Form 3231 at the time of registration. Inoculations for MMR, Varicella, and Hepititis B must have begun. Transfer students from within Georgia may have 30 days for their records to arrive.

4. Ear, Eye, and Dental Screening (Georgia Form 3300) ¡V At Enrollment
5. Guardianship/Custody orders ¡V At Enrollment
If student does not live with natural parents
6. Permanent Academic/Attendance Records from Previous School - 30 days
Your child's placement will be temporary until official records arrive from your previous school. The new school will request these records, but the responsibility for their receipt rests with the parents.
7. Proof of Residency ¡V At Enrollment
Utility deposit, lease agreement, contract, etc.
8. Copy of Social Security Card or Waiver ¡V At Enrollment
Admission
• Pre-K. A child is eligible for admission to Pre-Kindergarten if he/she reaches the fourth birthday on or before September 1 of the current school year.
• Kindergarten. A child is eligible for admission to Kindergarten if he/she reaches the fifth birthday on or before September 1 of the current school year.
• First Grade. A child is eligible for admission to first grade if he/she reaches the sixth birthday on or before September 1 of the current school year.
Out-of-state Transfers, Grades K and 1
A child who was a legal resident of one or more other states for a period of two years immediately prior to moving to Georgia and who was legally enrolled in a public kindergarten or first grade, or a kindergarten or first grade accredited by a state or regional association, shall be eligible for enrollment if the child will attain the age of five for Kindergarten or six for first grade by December 31 of the fiscal year and is otherwise qualified.
Any child who completes Kindergarten under this policy shall be eligible for enrollment in first grade in the subsequent school year. Also, any child who met the conditions above who entered Georgia and completed Kindergarten shall be eligible for first grade the subsequent year if such child attains the age of six by December 31 of that fiscal year.
