Begins October 10, 2007, and ends on November 9, 2007 (at 4:00 p.m.)

The annual open enrollment period is for active employees to review current benefits and make new elections for the upcoming Plan Year. If you’re already an employee, review the information provided under this section and then click on the Online Employee Benefits link above. If you need assistance, call the Employee Benefits Office.

State Health Plan Links - Provider Directories
PPO In/Out-of-State
By treating with a network PPO physician, you receive the highest benefit available under the PPO option. Benefits are payable if you must see an out-of-network physician, however, benefits are reduced.
BlueChoice HMO
Health Maintenance Organizations (HMO’s) require treatment by network physicians. If you select BlueChoice HMO, you must select a Primary Care Physician (PCP). Your medical treatment will be provided by your PCP and if at anytime you require treatment by a specialist, your PCP will make a referral on your behalf. Review this Directory to select a PCP and confirm which specialists participate.
Cigna HMO
United HealthCare HMO
United HealthCare HMO does not require you to see a Primary Care Physician before going to network specialists. Select any physician from the United HealthCare directory for medical treatment.
